When using tadalafil to manage pulmonary arterial hypertension, it is vital to avoid taking Cialis® or other phosphodiesterase type 5 (PDE5) inhibitors such as sildenafil (Revatio® or Viagra®) or vardenafil (Levitra®). The reason for this caution is that Cialis® also contains tadalafil, which can lead to an increased risk of adverse effects if these medications are combined or if you exceed the recommended dosage of tadalafil.
Should you experience an erection lasting longer than 4 hours or an erection accompanied by pain lasting more than 6 hours, you must seek immediate medical attention. This situation, known as priapism, is serious and requires prompt treatment to prevent long-term damage to the penile tissue.
This medication does not provide any protection against sexually transmitted infections (STIs), including HIV or AIDS. It is important to practice safe sex and consult your healthcare provider if you have any inquiries regarding safe sexual practices.

Additionally, some recreational drugs known as "poppers" (like amyl nitrate, butyl nitrate, or nitrite) also contain nitrates and should be avoided. If you require nitrate therapy, ensure that at least 48 hours have elapsed since your last dose of tadalafil before taking the nitrate.
Limit your alcohol consumption while taking tadalafil. High alcohol intake (such as five or more glasses of wine or five or more shots of whiskey) can heighten your risk of adverse effects like headaches, dizziness, increased heart rate, or lowered blood pressure.
Contact your physician immediately if you notice a sudden loss of vision in one or both eyes. This side effect requires urgent medical evaluation.
It is equally important to consult your doctor if you experience a rapid decline in hearing or complete loss of hearing, which may be accompanied by symptoms such as dizziness and tinnitus (ringing in the ears).
